Student Code of Conduct Incident/Distressed or Dangerous Student Report

This form is to be used in two ways.

Student Code of Conduct Violations: All potential Student Code of Conduct violations should be reported using this form. Submission of this form to the Vice President for Student Success will initiate a student conduct investigation.

Distressed/Dangerous Student: Any behavior or situation – whether or not it involves a potential Student Code of Conduct violation – that leads you to believe a student is in distress or a danger to himself/herself or the college community should be reported via this form. Submission of this form to the Vice President for Student Success will initiate the review process, which could include convening of the College’s Crisis Management Team to determine the proper course of action to assist the student and/or safeguard the College community. This review will include the participation of mental health and law enforcement professionals on an as-needed basis.
